INSTRUCTION  FOR  COMPLETING  THE  REGISTRATION  FORM

Please write in block letters and indicate your name as you would like it to be given in the name tag.

Use only one form to register one Lion and his / her spouse and if more than one Leo or Guest, please write the name in space provided at the bottom of the application form.

Cheques should be drawn in favour of “Lions  District 306 B2 Convention 2011” and crossed A/C payee. No cash will be accepted.

All registrations will be acknowledged by a registration number slip which should be produced to collect the docket.

The registration desk will be open to Lions, the day prior to the convention at the convention site for registration and collection of dockets.

Please contact your club president for club wise registration.

400 gifts will be issued by attendance committee on first come first serve basis for those who come with the prior registration to the convention site.

Duly completed form and the relevant payment by cheque should be sent to.
